29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:  a.  On or about July 7, 2015, at the above addressed jobsite, employees were required by the employer to wear a tight fitting respirator when working with silica dust.  The employer failed to establish and implement a respiratory protection program which included, but was not limited to medical evaluations, fit testing, proper maintenance and training.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:  a.  On or about July 7, 2015, at the above addressed jobsite, employees were exposed to silica when working with concrete dust.  The employer failed to develop and implement a written hazard communication program thereby exposing employees to hazards associated with crystalline silica exposure.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:  a.  On or about July 7, 2105, at the above addressed jobsite, employees were walking and working around exposed electrical wires which were protruding out from uncovered electrical boxes, thereby exposing employees to electrical hazards.
