29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) For the establishment at 4820 Middle Road, Sarasota, FL 34234, as occurring on or about 07/06/2015, employees removing ridge vents on a metal roof were not protected from falling through skylights. On 07/06/2015, an employee fell 28 ft. and 10 in. through an unguarded skylight. The employee was not wearing a full body harness, and the skylight was not protected with covers or guardrails.

Employees who installed anchor points were not aware of the weight capacity that the anchor point had to hold. b) Employees used exhaust vents as anchor points, and these vents are not designed to support a 5000 pound load.
