29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. a) Shipping Department, on or about 6/25/2015: Employee was retrieving a 24-inches x 24-inches wood pallets from underneath the cantilever rack system which had the right side anchor leg unguarded causing the employee to trip and fall fracturing his right hip. b) Shipping Department, on or about 7/9/2015: Employees were retrieving 24-inches x 24-inches wood pallets from the underneath the cantilever rack system which had various anchor legs unguarded. Employees are exposed to trip and fall hazards.
