29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:    The employer does not ensure stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were equipped with at least one handrail and one stair rail system along each unprotected side or edge. This violation occurred on or about July 9, 2015, at the construction site where employees were exposed fall hazards while using a stairway without a handrail or stair rail.    Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, are equipped with at least one handrail and one stair rail system along each unprotected side or edge.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  The employer does not ensure that the path to ground from circuits, equipment, or enclosures is permanent and continuous. This violation occurred on or about July 9, 2015, at the construction site where employees were exposed to electrical hazards when using a Porter Cable Air Compressor SN: 2967867682 that was missing the grounding pin.      Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the path to ground from circuits, equipment, or enclosures is permanent and continuous.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  The employer does not ensure that flexible cords are connected to fittings so that tension would not be transmitted to joints or terminal screws. This violation occurred on or about July 9, 2015, at the construction site, where employees were using an extension cord without strain relief between the cord and the female plug end on the cord.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that strain relief is provided on flexible cords and cables.
