29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: a. On or about July 7, 2105, at the above addressed jobsite, employees and subcontractor employees were walking and working around exposed electrical wires which were protruding out from uncovered electrical boxes, thereby exposing employees to electrical hazards.

29 CFR 1926.1101(d)(5): All general contractors on a construction project which includes work covered by this standard shall be deemed to exercise general supervisory authority over the work covered by this standard, even though the general contractor is not qualified to serve as the asbestos "competent person" as defined by paragraph (b) of this section. As supervisor of the entire project, the general contractor shall ascertain whether the asbestos contractor is in compliance with this standard, and shall require such contractor to come into compliance with this standard when necessary. a. On or about September 3, 2015, at the above addressed jobsite, a subcontractor performing demolition work uncovered friable asbestos containing thermal system insulation at a multi-employer work site. The general contractor did not require the asbestos containing material be encapsulated and/or abated, thereby exposing contractors to hazards associated with asbestos exposure.

29 CFR 1926.1101(l)(4): Waste and debris and accompanying dust in an area containing accessible thermal system insulation or surfacing ACM/PACM or visibly deteriorated ACM: a. On or about September 3, 2015, at the above addressed jobsite, a contractor exposed friable thermal system insulation during demolition work. The waste, debris and dust in the area of the ACM was not cleaned up or disposed in leak tight containers thereby exposing employees and subcontractor employees to hazards associated with asbestos exposure.
