29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: Employees operating the Flexibend were exposed to amputation hazards as employees were not protected from placing their hands between the upper and lower die because the point of operation was not guarded to prevent employees from reaching into the area during operation.
