29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about July 9, 2015, on the south side of a new commercial structure, employees working beneath employees on a second level were not wearing hard hats, exposing employees to the hazard of being struck by falling debris.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.  On or about July 9, 2015, on the southeast side of a commercial building, an employee working on the framed structure was not protected from a 14 feet fall hazard by the use of a personal fall protection system.  b.  On or about July 9, 2015, on the northeast side of a commercial building, an employee working on the framed structure was not protected from a 12 feet 7 inch fall hazard by the use of a personal fall protection system.
