29 CFR 1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support: Jobsite: 8000 Hall Street, St. Louis, MO 63137 On or about June 23, 2015 a fiberglass Louisville extension ladder, m/n FE3232, was not adequately secured at the top support located approximately 21 feet above ground. As a worker was descending the ladder, the ladder slipped from the upper support and fell sideways down the vertical wall.
