29 CFR 1926.755(b)(1): Anchor rods (anchor bolts) were field-modified without the approval of the project structural engineer of record: Location: 535 Zerega Ave. On or about: 5/19/15 a) Numerous anchor rods were field modified without the approval of the structural engineer. Anchor rods were not imbedded in to the concrete foundation to the depth specified in the engineering blueprints.

29 CFR 1926.755(b)(2): Prior to the erection of columns, the controlling contractor did not provide written notification to the steel erector if there has been any modification of the anchor rods (anchor bolts) of those columns: Location: 535 Zerega Ave. On or about: 5/19/15 a) Numerous anchor rods were field modified without the approval of the structural engineer. Anchor rods were not imbedded in to the concrete foundation to the depth specified in the engineering blueprints. Written notification was not provided to the steel erector of field modifications prior to steel erection.
