29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: a) Hinged metal covers were missing from one or both dual-port floor exhaust intakes throughout the repair bays in the service shop; leaving 6" X 8" openings in the floor that were covered by flexible rubber mats that were not of standard strength. Condition(s) were noted on or about 7/8/2015.
