Bridgeport Area:  On, or about, July 13, 2015, a white pedestal fan provided for general ventilation was not designed for use in an industrial location in that it was not provided with a three wire hard service or junior hard service (round) cord.    C.  Saw Area:  On, or about, July 13, 2015, three power taps were used in series and in combination with several extension cords to energize equipment such as, but not limited to:  a Ridgid shop vac, a computer, a battery charger, a radio, the Rong Fu bandsaw and an overhead light fixture located on the mezzanine.

Saw Area:    1.  A blade guard was not provided on the unused portion of the blade on the Cosen Model MH-1016JA horizontal band saw. 2.  The Rong Fu horizontal band saw, Model S92285, was being used as a vertical bandsaw.  The spoked drive pulleys for the saw blade were exposed as well as all of the non-working portion of the blade.     B.  CNC Area:  The following machines were not provided with operable door interlocks in the automatic mode:  1.  CNC Lathe #1:  A Yang SML-20 CNC Turning Center. 2.  CNC Lathe #2:  A Yang SML-30 CNC Turning Center 3.  VMC#1:  A Yang SMV-1000 CNC milling center.  4.  VMC #2:  A Yang SMV-1000 CNC milling center. 5.  VMC #3:  A Yang SMV-1000 CNC milling center.  bmi CAD Services, Inc. was previously cited for a violation of an equivalent occupational safety and health standard [1910.212(a)(3)(ii)], which was contained in OSHA Inspection Number 314405101, Citation Number 1, Item Number 2b and was affirmed as a Final Order on July 26, 2011, with respect to a workplace located at Simsbury, Connecticut.
