29 CFR 1910.147(c)(4)(i):      Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:      (a) On or about July 8, 2015 at worksite 2662 Militia Dr , Jefferson City, Missouri. employees were exposed to serious amputation, crushing and struck-by injuries, in that the employer had not developed specific energy control procedures for the shutting down and return operations for the  Ultra/TPK Palletizer machine on line 2 in the manufacturing area.       Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.147(c)(6)(i):    The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection    (a) On or about July 8, 2015 at worksite 2662 Militia Dr , Jefferson City, Missouri. An examination of Lockout/Tagout program elements determined that periodic inspections of energy control procedures were not accomplished and certified.  Specifically, reviews were not conducted to determine if listed steps were still adequate.     Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      (a) On or about July 8, 2015 at worksite 2662 Militia Dr , Jefferson City, Missouri. employees in the manufacturing area were exposed to a rotating parts hazard  when operating the Ultra/TPK Palletizer machine on line 2 , in that the Palletizer Machine a robotic arm was not guarded to prevent entry in the machine operating radius, or protected to stop rotating parts from contacting the operator.         Abatement certification for this item is required; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
