Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection (a) On or about July 8, 2015 at worksite 2662 Militia Dr , Jefferson City, Missouri. An examination of Lockout/Tagout program elements determined that periodic inspections of energy control procedures were not accomplished and certified. Specifically, reviews were not conducted to determine if listed steps were still adequate. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
