29 CFR 1910.23(d)(1)(i): On stairways less than 44 inches wide having both sides enclosed, at least one handrail, preferably on the right side descending.  (a) Warehouse building: On or about July 9, 2015, the employer failed to provide handrails on stairs in the warehouse building. Employees were exposed to fall hazards.      (1) Stairs located at the North end of the section of the warehouse building housing the shipping, quilting, photography, and customer service departments did not have handrails installed.      (2) Stairs located at the South end of the section of the warehouse building housing the shipping, quilting, photography, and customer service departments did not have handrails installed.

29 CFR 1910.23(d)(1)(v): On stairways 88 or more inches wide, one handrail on each enclosed side, one stair railing on each open side, and one intermediate stair railing located approximately midway of the width.   (a) "Library" Building formerly housing the quilting department: On or about May 20, 2015, the employer failed to provide an intermediate stair railing located approximately midway of the width of the stairs outside of the "library" building. Employees were exposed to fall hazards while accessing the building. One employee was severely injured due to falling down the stairs.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENATY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.24(e): "Angle of stairway rise." Fixed stairs must be installed at angles to the horizontal of between 30 deg. and 50 deg. Any uniform combination of rise/tread dimensions may be used that will result in a stairway at an angle to the horizontal within the permissible range.   (a) "Library" Building formerly housing quilting department: On or about May 20, 2015, the employer failed to provide stairs installed at angles to the horizontal of between 30 degrees and 50 degrees.      (1) The top three steps of the stairs outside the "library" building produce an angle to the horizontal of approximately 20 degrees. Employees including Long Arm Quilters were exposed to trip, falls and slip hazards. One employee was severely injured due to falling down the stairs.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENATY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19

29 CFR 1910.24(f): "Stair treads." All treads must be reasonably slip-resistant and the nosings shall be of nonslip finish. Welded bar grating treads without nosings are acceptable providing the leading edge can be readily identified by personnel descending the stairway and provided the tread is serrated or is of definite nonslip design. Rise height and tread width must be uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs.  (a) "Library" Building formerly housing the quilting department: On or about May 20, 2015, the employer failed to provide stairs of uniform rise height and tread width throughout a flight of stairs.      (1) Stairs outside the "library" building are not uniform in either rise height or tread width throughout the flight of stairs. Employees including Long Arm Quilters were exposed to slip, fall, and trip hazards. One employee was severely injured due to falling down the stairs.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes must have faceplates installed so as to completely cover the opening and seat against the finished surface.  (a) Near North stairs in Warehouse Building: The employer failed to install a faceplate to cover the opening around a snap switch located next to the stairs at the North end of the warehouse building housing the shipping, quilting, photography, and customer service departments. Employees were exposed to electric shock and fire hazards.

29 CFR 1904.39(a)(2):  Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  (a) Missouri Star Quilt Co.: The employer failed to report the in-patient hospitalization of an employee to OSHA within twenty-four (24) hours. On  May 20, 2015 an employee was transported to the emergency room at Cameron Regional Medical Center Hospital and transferred to St. Luke's Hospital and hospitalized for treatment of a concussion, multiple broken bones, and nerve damage.
