29 CFR 1926.451(g)(1): Each Employee on scaffolds more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Exterior supported scaffold at front entrance of house:    An employee was working on the 2nd tier platform of a 4-tier-high supported scaffold, and was not protected from falling to lower levels by the use of fall protection systems. The employee was exposed to falling approximately 12 feet. On or about 07/08/15.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.     a) 2nd floor, stones on window lintels:    The employer did not conduct an engineering survey of the window lintels made of stones to determine the possibility of unplanned collapse of the damaged stones prior to permitting employees to start demolition operations. A damaged stone collapsed and fell to the street level during demolition operations. On or about 07/08/15.
