29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a. Site - Employees performing roofing work from a flat roof were not protected from falls of approximately 30 feet when the warning line system on site was not set up and no other fall protection was being used. R.J. Kellogg Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29CFR 1226.502(b)(1), which was contained in OSHA inspection number 976098, citation number 001, item number 001 and was affirmed as a final order on 7/5/2014, with respect to a workplace located at Anderson Hall, South Main Street, Wolfeboro, NH 03894.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. Site - Employees were not protected from falls of approximately 10 feet when the fiberglass extension ladder that was positioned inside a roof hatch in a way that required employees to climb on the rafters to get between the roof and the ladder.
