OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a. Warehouse - metal storage racks used to store material and product were not bolted/anchored to the floor; therefore exposing employees to a possible struck-by hazard in the event the storage racks were to topple.  Violation observed on or about 7/10/15.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toe board wherever, beneath the open sides:    a. Break room ceiling - the ceiling area of the break room accessed by stairs was used as a working surface and did not have guard rails along the edges which exposed employees to up to a nine feet fall hazard.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:    a. An employee was injured and admitted to the hospital on or about July 1, 2015.  The employer was aware of the in-patient hospitalization and did not report the in-patient hospitalization to OSHA.
