29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On June 23, 2015, employees were exposed to a 19.7 foot fall hazard while walking/working on the roof of a commercial building with a flat roof and the employer did not ensure employees were utilizing fall protection. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On June 23, 2015, employees were exposed to a 19.7 foot fall hazard through unprotected skylights while performing work on the roof of a commercial building and the employer did not ensure employees were adequately protected. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
