29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. In the Production areas, on or about July 14, 2015, employees conducted servicing and maintenance to six machines on a weekly basis and the employer did not have an established program consisting of an energy control procedures, employee training and periodic inspections for machines or equipment; such as but not limited to: the Callahan brand Radio Frequency (RF) packaging machine, the Alloyd brand RF packaging machine, the Alloyd brand blister pack machine, the two (2) Visual Packaging brand blister pack machines and the HFT brand RF packaging machine, thereby exposing employees to unexpected energizing, startup or release of stored energy equipment hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a. In the Janitorial Supply Room and Maintenance Shop, on or about July 14, 2015, employees worked with, and in close proximity to, hazardous and flammable chemicals such as oils, chlorides, acids, alcohol, ethanol, hypochlorite, propane, and gasoline with exposure to serious injury and illnesses and the employer had not developed nor implemented a written hazard communication program.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a.  In the Janitorial Supply Room and Maintenance Shop, on or about July 14, 2015, employees worked with, or around, hazardous and flammable chemicals such as oil, chloride, acid, alcohol, ethanol, hypochlorite, propane, and gasoline and the employer did not provide information of the health and physical hazards of these chemicals at initial hiring assignment and or whenever a new chemical hazard was introduced into their work area.
