29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: rotating bagel slicer cutting blade. On or about July 5, 2015, At the job site, 315 Southwind Place, Manhattan, Kansas, an employee was operating the bagel slicer after cleaning it without the top metal blade guard in place. Employee was exposed to amputation hazard from the rotating blade.
