29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections. The most recent occurrence was at 6162 Waterman Boulevard, St. Louis, MO 63112, where the employer failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards. Two carpenters were exposed to an approximate 25 foot, 6 inch fall hazard at the rear of the residential structure as they worked to construct guardrail systems on the third level deck without the use of an adequate fall protection system and/or fall arrest system. An additional carpenter was exposed to an approximate 17 foot fall hazard as he was assisting to install the guardrail system while working from a ladder positioned on the second level deck without the use of an adequate fall protection system and/or fall arrest system.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): The employer permits workers to be exposed to fall hazards created during residential construction activities. The most recent occurrence was at 6162 Waterman Boulevard, St. Louis, MO 63112, where two carpenters were exposed to an approximate 25 foot, 6 inch fall hazard at the rear of the residential structure as they worked to construct permanent guardrail systems on the third level deck without the use of an adequate fall protection system and/or fall arrest system. An additional carpenter was exposed to an approximate 17 foot fall hazard as he was assisting to install the guardrail system while working from a ladder positioned on the second level deck without the use of an adequate fall protection system and/or fall arrest system.
