29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.    (a) Location - 23 North Atkinson Street, Newburyport, MA:  On or about 7/13/2015, employees working from a ladder jack scaffold platform that was 18 feet 6 inches above the ground were not protected from falling to the ground below.

(a) Location - 23 North Atkinson Street, Newburyport, MA:  On or about 7/13/2015, employees were not protected from falling while working on a roof that was 19 feet above the ground below.
