29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a. 236 Hightower Parkway, Dawsonville, GA (Jobsite) - Employees were exposed to fall and/or overturning hazard. Employees (forklift operators) were using the lift personnel to retrieve storage material from the upper racks approximately 13ft. above ground level. The employer did not obtained prior written approval from the manufacturer and/or qualified register engineer, on or about July 13, 2015.
