29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Shopping center: On or about July 13, 2015 and at times prior thereto, an employee was working form an aerial lift without any form of fall protection. This condition exposed the employee to a fall greater than 10-feet to the ground below.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: Construction Site: On or about July 13, 2015, the employer did not provide a training program to enable employees to recognize the hazards of falling. This condition exposed the employees to a fall greater than 10-feet to the ground below.
