29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Roadway, Corner of Lower Westfield Road and Holyoke Street, Holyoke, MA: On or about July 13, 2015: employees were observed working in an excavation greater than 5 feet deep and were not protected from cave in's by an adequate protective system. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
