OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:  (a) On or about 07/10/2015 - at the Lower Shop, the anchorage was not capable of supporting at least 5,000-pounds per employee attached using a personal fall arrest system when working on the top of pumper and ladder fire trucks exposing the employee to varying fall hazard heights of about 8 to 11-feet.  (b) On or about 07/10/2015 - at the Upper Shop,  the anchorage location allowed a swing radius and/or free fall distance of more than 6-feet for employees exposed to varying fall hazard heights of about 8 to 11-feet while working on the top of pumper and ladder fire trucks and using a personal fall arrest system with a 20-ft self-retracting lifeline.
