29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:   a) 22 Old Forge Road, Jonestown, PA / Tire Repair Building; Employees were building a steel fabricated addition while working in an aerial lift approximately 13 feet above the ground without the use of fall protection, on or about 07/14/2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.752(b): The steel erection contractor began steel erection work without written notification that the concrete in the in the footings, piers and wall, or the mortar in the in the masonry piers and walls has attained, on the basis of an appropriate ASTM standard test method of field-cured samples, either 75 percent of the intended minimum compressive design strength or sufficient strength to support the loads imposed during steel erection:   a) 22 Old Forge Road, Jonestown, PA / Tire Repair Building; The employer began erecting steel without having received written notification that the concrete in the footings had been field-cured to 75% design strength, on or about 07/14/2015.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
