29 CFR 1910.95(c)(1): The employer shall administer a continuing, effective hearing conservation program, as described in paragraphs (c) through (o) of this section, whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level (TWA) of 85 decibels measured on the A scale (slow response) or, equivalently, a dose of fifty percent. For purposes of the hearing conservation program, employee noise exposures shall be computed in accordance with appendix A and Table G-16a, and without regard to any attenuation provided by the use of personal protective equipment.    a. Incoming Plastic Conveyor Line # 2 - A line sorter was exposed to continuous noise at 198.2 % of the permissible daily dose level, when measured with 80 dBA threshold, or an average sound level of the approximately 94.93 dBA for the 318 minute sampling on July 21, 2015. Exposure calculation included zero increment for the 162 minutes not sampled.      b. Incoming Plastic Conveyor Line #'s 1,2,3 - A line sorter was exposed to continuous noise at  185.0 % of the permissible daily dose level, when measured with the 80 dBA threshold, or an average sound level of the approximately 94.43 dBA for the  311 minute sampling on July 21, 2015. Exposure calculation included zero increment for the 169 minutes not sampled.     c. Main Floor - A machine operator/fork lift truck operator was exposed to continuous noise at 134.2 % of the permissible daily dose level, when measured with the 80 dBA threshold, or an average sound level of the approximately 92.12 dBA for the 253 minute sampling on July 21, 2015. Exposure calculation included zero increment for the 227 minutes not sampled.     d. West side of the Building - A fork lift truck operator was exposed to continuous noise at 88.5 % of the permissible daily dose level, when measured with the 80 dBA threshold, or an average sound level of the approximately 89.11 dBA for the 288 minute sampling on July 21, 2015. Exposure calculation included zero increment for the 192 minutes not sampled.    Yet, the employer had not establish a complete and effective hearing conservation program.

29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent ) was not completed in detail as required by the regulation:  a. Front Office - On or about July 16, 2015, the information required for column "F" was not completed in detail as required by the regulation. The missing information on the year 2015's OSHA form 300 log was the following;  1. For entry # 1 - on 5/18/15, the injury or illnesses description was missing.  2. For entry # 3 - on 6/20/15, the object/substance that directly injured or made person ill was missing.  3. For entry # 4 - on 6/19/2015, the parts of body affected and object/substance that directly injured or made person ill were missing.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard");   a. Recycling Activities Area - On or about July 16, 2015, the employer had made available the voluntarily use of filtering masks and he had not informed employees of the Appendix D of the respiratory standard.
