Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while working from a 16 foot wide platform elevated by a telehandler: (a) On or about July 14, 2015 for an employee exposed to fall hazards of approximately ten feet while working from a work platform, approximately 16 feet wide, attached to the forks of a Carelift ZB6042-44 Telehandler at 1723 Central Loop in Aberdeen, South Dakota. Abatement Note: Among other means, feasible and acceptable methods to correct the hazard would be to follow ASME B56.6b - 1992 Safety Standard for Rough Terrain Forklift Trucks including but not limited to: 1. Use a platform with a width that does not exceed ten inches beyond the overall width of the truck. 2. Use an alternative means to elevate employees to working levels.
