Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On July 16, 2015, at the JBPHH Utah Hall, an employee performing housekeeping activities near an unprotected edge, was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems, exposing the employee to a fall hazard of 12 feet onto the concrete surface.

29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors: a) On July 16, 2015, at the JBPHH Utah Hall, an employee utilizing a Stinger Wet/Dry Vacuum, was not protected against electric shock from faults in electrical devices by a ground fault circuit interrupter or an assured equipment grounding conductor program.
