29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) On or about July 15, 2015 an employee of R/J Group, Inc. was working in an aerial boom lift 30 feet above the ground to remove scrap materials from the roof and lower them to the dumpster below, the operator was using the lift to carry roofers to and from the upper roof structure, and although was wearing a fall protection harness he was not tied off or latched to the boom or basket of the lift.

On or about July 15, 2015 Five employees were working on a 3 story 5:12 pitch residential roof installing a clay tile roof and were exposed to fall hazards in excess of 30 feet because they were not protected by any means of a Gaurdrail, Safety Net, or personal fall protection system.

Employer did not properly record the fatal injuries of a R/J Group Inc employee on the 2015 OSHA 300 logs.
