(B) The name and signature of the person who conducted the inspection and the date. On or about July 15, 2015 and prior thereto, employees were exposed to struck-by hazards, when a daily inspection of the National Crane NBT 40 serial #298019 was not documented and maintained as required, while the crane was lifting and suspending an air condition unit onto the roof of the building.
