29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 2580 Fair Way Road, York, PA - Employee(s) were observed performing roofing work activity on a 6 in 12 pitch residential roof without the benefit of a fall protection system, thereby exposing employees to a fall of approximately 8 feet 10 inches to 12 feet to the ground below, on or about June 2, 2015.         No abatement certification or documentation required.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a)  2580 Fair Way Road, York, PA  - The employer did not provide training to employee(s) which would have allowed employee(s) to recognize the hazards of falling and the equipment and procedures to use to minimize those hazards, thereby exposing employees to possible injuries, on or about July 2, 2015.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(4):  Ladders were used for purposes other than the purpose for which they were designed:    (a)   2580 Fair Way Road, York, PA  - The employees were using a 4 foot Werner fiberglass step ladder in the closed position leaning up against the main roof being used for access from the ridge of the garage roof, thereby exposing the employee to injuries from ladder deflection, on or about March 18, 2015.    No abatement certification or documentation required.
