29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a)  On or about July 15, 2015, employees were exposed to falls because the employer did not ensure that a ladder or some other safe means was used for egress from a 9 feet deep trench.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:  a)  On or about July 15, 2015, and at times prior, employees were exposed to injuries from falling excavated materials while working in a trench.  The materials excavated from a 15 feet deep trench were piled at the edge of the trench and were not moved at least 2 feet from the edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  a)  On or about July 15, 2015, and at times prior, employees were exposed to cave-ins while they worked in an excavation which was not protected from cave-ins by an adequate protective system.  Employees worked in an excavation which was approximately 15 feet deep.  The excavation had straight cut walls with an inadequate shielding system (trench box) that was too small for the depth of the excavation.  Employees worked inside and outside of the shielding system.       b)  On or about July 15, 2015, employees were exposed to cave-ins while they worked in an excavation which was not protected from cave-ins by an adequate protective system.  Employees worked in an excavation which was approximately 9 feet deep.  The excavation had straight cut walls and no cave-in protective system was used.

29 CFR 1926.651(k)(2): Where a competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions were taken to ensure their safety:    a)  On or about July 15, 2015, and at time prior, employees were exposed to cave-in hazards because the employer did not take precautions to remove employees that worked inside of two trenches (15 feet and 9 feet deep) which had straight cut walls and inadequate cave-in protection.  Even after several small cave-ins in the deeper trench, the employer allowed employees to continue working in it.
