OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to physical harm such as a falling hazard:    The employer failed to ensure employee are protected from being crushed. This hazard was most recently documented on July 16, 2015, at 1221 S. 203rd St., Omaha, Nebraska when a employee on a A-frame stepladder, working off of the top two rungs fell, causing a skull fracture.        AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE:        1)   Train employees to use a stepladder in accordance with the manufacturer's instructions.       2)   Develop work rules and disciplinary procedures prohibiting working above the top two rungs of any stepladder.    3) Utilize and implement the guidelines as outlined in the American National Standard for Ladders  Portable Reinforced Plastic  Safety Requirements ANSI A14.5 - 2000.  This includes that the user shall be familiar with all usage of fiberglass ladders. This includes, but is not limited to:    a) Climbing and working locations. The user shall climb or work with the body near the middle of the step or rung. The user shall not step or stand higher than the step or rung indicated on the label marking the highest standing level of a ladder.
