29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: a) At 1461 Kin Ark Ct in St. Louis, MO 63132 (plant) - On or about 07/16/2015, employees were exposed to elevated fall hazards of approximately 10 feet into a concrete pit where a standard railing did not have a top rail.

29 CFR 1910.36(g)(4): 29 CFR 1910.36(g)(4): Objects that projected into the exit route reduced the width of the exit route to less than the minimum width requirements for exit routes: a) At 1461 Kin Ark Ct in St. Louis, MO 63132 (plant) - On or about 07/16/2015, employees were exposed to delayed egress hazards where a wooden zinc shield, made from 4'-by-8' sheets of plywood and 2"-by-4" framing lumber, reduced the width of the exist route to approximately 19 inches.
