Worksite located at 2407 S. 291 Highway, Independence, Missouri- The employer had field modified the aerial lift without written approval from the manufacturer. An employee was observed standing and working on the modified section of the lift.

Worksite located at 2407 S. 291 Highway, Independence, Missouri- Employee was observed climbing in and out of the aerial lift using the guardrails of the lift. Employee could fall out of the lift or into the lift and cause serious trauma.

29 CFR 1926.1053(a)(8): A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used: (a). Worksite located at 2407 S. 291 Highway, Independence, Missouri- Employee was using a stepladder that had its' metal spreader collapsed and was leaning up on the wall of the structure in which an employee could fall due to the ladder slipping out from underneath him.
