29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 15, 2011, and at times prior thereto, at the trash compactor area a trash compactor was being operated which was not provided with either an inter-locking device or a dead-man switch device. Employees were exposed to hazards of being crushed by the machine.

29 CFR 1910.304(b)(2): Outlet devices had an ampere rating less than the load being served: On or about July 15, 2015, or at times prior thereto, in the break room two microwave ovens and a toaster were plugged into the same outlet device which had a current rating less than the load being served. This exposed employees to fire hazards.
