On or about July 13, 2015, at the roof top, employees using Hitachi nail guns were not wearing safety glasses being exposed to eye injuries.

29 CFR 1926.501(b)(13): 29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On or about July 13, 2015, roof area, employees installing roofing shingles on a 4:12 pitch roof were not protected from falling 18 feet 7 inches by the use of a guardrail system, safety net system, personal fall arrest system or any other type of fall protection method.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a. On or about July 13, 2015, back of building, the Werner 16-foot extension ladder was only extended 12 inches past the upper level landing and was not secured to prevent accidental displacement exposing employees to a 18 feet 7 inches fall hazard.
