29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On and before July 16, 2015, at a worksite located at 452 East Warner Road, Akron, OH 44319, during preparation work for the 2015 Bridgestone Invitational including but not limited to scaffolding erection, the employer did not ensure all employees were properly trained to recognize the hazards associated with work on and from various types of scaffold including tube and coupler and mobile scaffolding.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: On and before July 16, 2015, at a worksite located at 452 East Warner Road, Akron, OH 44319, during preparation work for the 2015 Bridgestone Invitational including but not limited to scaffolding erection, the employer did not ensure employees were provided re-training in the recognition and avoidance of unsafe conditions while using ladders. Employees were observed during the inspection using A-frame step ladders without extending the spredder bar and standing on the upper steps.
