The roof had a pitch of approximately 8 on 12. The employees were not utilizing any fall protection such as but not limited to personal fall arrest system, guardrails and safety nets. Conditioned observed on or about 07/17/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. Employees on site had not received fall protection training from the company. Condition observed on or about 07/17/15. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
