OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a tip over/crushing hazard due to the possibility of overloading the JLG 460J Telescopic Aerial Boom Lift and causing it to tip over.  a. On or about July 17, 2015 - at the above addressed jobsite,  a JLG 460J Telescopic Aerial Boom Lift was used to hoist a wooden cross using a cable wire sling which was affixed to the bottom of the basket in conjunction with lifting straps placed around the cross structure.  Employees were exposed to hazards associated with overloading and failure of the lift.
