29 CFR 1903.19(c)(3): The employer did not submit certification that abatement is complete for each cited violation, in addition to the information required by paragraph (h) of this section, the date and method of abatement and a statement that affected employees and their representatives have been informed of the abatement.  On or about November 24, 2015, the employer failed to submit abatement certification for the following citation issued on November 4, 2015;  1. Citation 1, Item 1a: 29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Location: 90 5th Avenue, Haverhill, MA  On or about 7/17/15, a worker on the ground without a hard hat was exposed to overhead struck-by hazards while workers on the roof above were installing new shingles.

Location: 90 5th Avenue, Haverhill, MA  On or about 7/17/15, workers operating pneumatic nail guns were not wearing safety glasses and exposed to eye injury hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 90 5th Avenue, Haverhill, MA  On or about 7/17/15, workers were exposed to fall hazards of greater than 19-feet, 9-inches while working from an 8-pitch roof without a fall protection system.
