29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about July 3, 2015, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing maintenance/servicing, set up and cleaning on machines such as, but not limited to, the Globe meat slicer. Failure to lock out these components while these tasks are being performed exposes employees to amputation injuries.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about July 20, 2015, the employer failed to report an employee amputation injury. On July 3, 2015, an employee cleaning a meat slicer sustained an amputation injury; the employer did not report the amputation within 24 hours.
