29 CFR 1926.451(b)(1)(i): Each platform scaffold plank was not installed so that the space between the planks, and the space between the planks and the uprights, was no more than 1 inch wide, except where the employer can demonstrate that a wider space was necessary:  (a)LC Plus Stucco LLC at 13987 Barbour St., Broomfield, CO 80023:  On and before July 18th, 2015, the employer allowed employees to color the stucco using multiple platforms consisting of only two 2" x 10" planks wide, even though the scaffold was designed to support platforms that were three 2" x 10" planks wide.  The condition exposed an employee to a fall hazards of approximately 15.25 feet.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  (a)LC Plus Stucco LLC at 13987 Barbour St., Broomfield, CO 80023:  On and before July 18th, 2015, the employer did not ensure that the scaffold was erected under the supervision of a competent person.  The condition resulted in a fall hazards of approximately 15.25 feet.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a)LC Plus Stucco LLC at 13987 Barbour St., Broomfield, CO 80023:  On and before July 18th, 2015, the employer allowed employees to use this scaffold even though it was missing mid rails on the east and north sides. The condition resulted in a fall hazards of approximately 15.25 feet.  Abatement note:  Cross-bracing can serve as a top rail or a mid rail, depending on it location, but not both.  An additional horizontal rail is required to provide adequate fall protection.  Reference 29 CFR 1926.451(g)(4)(xv):  Crossbracing is acceptable in place of a midrail when the crossing point of two braces is between 20 inches and 30 inches above the work platform,  or as a toprail when the crossing point of two braces is between 38 inches and 48 inches above the work platform. The end points at each upright shall be no more than 48 inches apart.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a)LC Plus Stucco LLC at 13987 Barbour St., Broomfield, CO 80023:  On and before July 18th, 2015, the employer allowed employees to use a portable mixer, model # 4011, for use in mixing cement, even though it had a frayed flexible power cord. The power cord was taped in a manner that had the potential to allow moisture to gather on the energized wires.  The condition resulted in exposure with up to 120 volt electrical shock hazard.
