29 CFR 1910.269(c)(2):  The job briefing did not cover at least the following subjects: hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements.      (a)   Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757:  On or before July 10, 2015, the job briefing that covered work procedures involved, energy-source controls, and personal protective equipment.  On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground.  This condition exposed employees to electric shock and/or an electrocution hazard.

29 CFR 1910.269(l)(5)(i):  Each employee, to the extent that other safety-related conditions at the worksite permit, shall not work in a position from which a slip or shock would bring the body into contact with exposed, uninsulated parts energized at a potential different from the body.      (a)  Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757: On or before July 10, 2015, an employee worked in a position from which a slip or shock would bring the employee's body into contact with exposed, uninsulated parts energized at a potential different from the employee's.  On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground.  This condition exposed employees to electric shock and/or an electrocution hazard.

29 CFR 1910.269(l)(5)(ii):  When an employee performs work near exposed parts energized at more than 600 volts, but not more than 72.5 kilovolts, and is not wearing rubber insulating gloves, being protected by insulating equipment covering the energized parts, performing work using live-line tools, or performing live-line barehand work under paragraph (q)(3) of this section, the employee shall not work from a position where he or she cannot reach into the minimum approach distance, established by the employer under paragraph (l)(3)(i) of this section.      (a)   Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757:  On or before July 10, 2015, an employee worked from a position where he could reach into the minimum approach distance when he performed work near exposed parts energized at 7.2 kV, phase to ground.  On July 10, 2015 an employee was electrocuted when he came into contact with the energized parts.  He was not wearing rubber insulating gloves, or was protected by insulating equipment covering the energized parts.  This condition exposed employees to electric shock and/or an electrocution hazard.

29 CFR 1910.269(m)(3)(ii):  All switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized shall be open. Such means inoperable, unless its design does not so permit, and such means shall be tagged to indicate that employees are at work.      (a) Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757:  On or before July 10, 2015, all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized were not open. The disconnect switches for Feeder 1 were located about a mile away and were not opened to deenergize the back feed to the South Woodrow Substation.  On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground.  This condition exposed employees to electric shock and/or an electrocution hazard.

(a) Morgan County Rural Electric Association at Washington County Roads 20 & E, Woodrow, CO 80757:  On or before July 10, 2015, automatically controlled switches that could cause the opened disconnecting means to close were not rendered inoperable, unless its design does not so permit.  The circuit breakers for Feeder 1 were located inside the substation adjacent to the work area and the automatic re-closers were not rendered inoperable.  On July 10, 2015 an employee was electrocuted when he came into contact with 7.2 kV, phase to ground.  This condition exposed employees to electric shock and/or an electrocution hazard.
