29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position: a) At 625 W Karsch Blvd in Farmington, MO 63640 - On or about 07/20/2015, employees were exposed to tip-over hazards while standing on the side guardrails of a "JLG" (Model: 2630ES) scissor lift, and were climbing in and out of the basket to access a steep-slope roof, approximately 20 - 30 feet above a lower concrete surface. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
