This was most recently documented in the following instances:        (1) On or about July 9, 2015 had not controlled explosive dust accumulation on elevated surfaces.  Testing found the dust to be explosive with a Kst value of 42.05 with a pressure ratio of 8.62.        Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. This was most recently documented in the following instance:         (1)  On or about July 9, 2015 employees were exposed to a continuous source of noise from the grinding and sanding operation of handles.  The background noise was tested and found to continually exceed 93 dBA when both sanders were operating and 89 dBA with just one sander was operated.
