29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  50 Brookwood Avenue, Carlisle Pennsylvania -  An employee was working on the ground cleaning up old shingles.  The employer did not ensure that the employee wore a hard hat when exposed to over head hazards, on or about July 17, 2015.  No abatement certification or documentation required, corrected during the inspection.

29 CFR 1926.451(b)(2)(i): Each ladder jack scaffold, top plate bracket scaffold, roof bracket scaffold, and pump jack scaffold was not at least 12 inches (30 cm) wide:  (a) 50 Brookwood Avenue, Carlisle Pennsylvania - Employees worked on roof bracket scaffolding installing new shingles.  The employer did not ensure that the platform was at least 12 inches wide, on or about July 17, 2015.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not ensure that employees working on roof bracket scaffolding wore fall protection when exposed to falls greater than 10 feet to the ground, on or about July 17, 2015.   No abatement certification or documentation required, corrected during the inspection.
