29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) Jobsite: The supported tubular welded frame scaffold, measuring approximately 33 feet in elevation and 5 feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. Employees were capping the face of the building from the outrigger of the 4th working platform, approximately 30 feet above ground level, and from the 5th working platform, approximately 33 feet above ground level.    Observed on or about 7/1/15.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.  a) Jobsite: Employees conducting masonry operations used a portable ladder on the 5th working platform of the supported tubular welded frame scaffold, approximately 30 feet above ground level, without securing the scaffold against the lateral thrust exerted by the ladder, securing the platform units to the scaffold, or providing other means to stabilize the ladder against unequal platform deflection.  Observed on/or about 7/01/15.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.   a) Jobsite: The employer did not provide a safe means of access from one level to another by use of portable ladders, hook-on ladders or other acceptable means. Employees climbed the scaffold supports to access the 5th working level.    Observed on or about 7/1/15.   DD Fox was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 956734, citation number 2, item number 2, and was affirmed as a final order on 7/11/2014, with respect to a workplace located at 418 Federal Street, Philadelphia, PA 19106.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) Jobsite: Employees conducting masonry operations from the 5th working platform of the supported tubular welded frame scaffold were exposed to fall hazards, approximately 33 feet above ground level, without the use of a fall protection system.    Observed on or about 7/1/15.   DD Fox was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 956734, citation number 2, item number 1, and was affirmed as a final order on 7/11/2014, with respect to a workplace located at 418 Federal Street, Philadelphia, PA 19106.
