29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Job site, 417 Thorn Avenue, Sewickley, PA: On or about July 18, 2015, during rebar installation activities in an excavation determined to range from between approximately 5 feet 2 inches to 5 feet 11 inches in depth, the employer did not ensure that appropriate cave in protection for employees was utilized. Excavation wall angles ranged from approximately 45 degrees to 65 degrees, the soil type for the excavation was determined to be class "C" soil, and no other form of cave in protection was used during this work.
